Effective 11/1/2006, the Beech Street Preferred Provider network (PPO) will be available for doctors and providers in addition to Alaska Regional Hospital.

Vision Service Plan (VSP) - To verify which eye care providers in your area are participating VSP providers, you can call VSP toll free at 1-800-877-7195 or you can visit their web site address at www.vsp.com

Utilization Management Organization

You may contact Qualis Health by calling 1-800-783-8606 (toll free). The health care professionals at Qualis Health provide the following services under the Plan:

• pre-certification reviews of proposed health care before the services are provided, and
• case management which determines if the course of care or treatment is Medically Necessary
